I, the parent/guardian of the student named above, have read the IPAC 2024-2025 contract and understand all of the requirements expected of me and my child. I also understand the consequences listed if this contract is broken by either myself or my child. I fully comply with all of the rules and regulations stated in the 2024-2025 contract. I understand that I am required to be present for all parent meetings. In the event that I am unavailable, I understand that I must have someone present on my behalf or communicate that 1 Hour before the scheduled meeting.

Photo/Video Release Waiver:
I acknowledge and consent to allow Integrity Performing Arts Center (IPAC Dance Studio) to use: photos and/or videos of my child/myself in publications (such as flyers/brochures), advertisements (such as newspapers or online ads), our studio’s website, or on social media sites (such as the studio’s facebook or instagram account). I understand that in some cases, my child and/or my name may be used. I further understand no financial compensation will be for use of these photos/videos.
